IMMUNIZATIONS
All new students for the Summer and Fall 2010 semesters
must be submitted to Health Services
by June 1, 2010
You can call or email questions regarding immunizations to:

You can mail or fax the form to the address/fax # listed below.

STUDENT HEALTH INSURANCE
All waivers for the Spring 2010 semester
must be submitted by February 10, 2010.
Open enrollment for the Pearce Pearce
health insurance will end on February 10, 2010.
Please go to the following website:
http://www.studentinsurance.com to apply for the waiver.
If you would like to enroll in the UNCA
Health Plan through Pearce and Pearce go the website identified
above and click the link for "Mandatory Enrollment".
